If you require any other information not available on the website you need to complete the Access to Information request form and email form to ppecbinfodesk@ppecb.com
Click here to download the Access to Information request form which you have to complete as part of our Information policy as guided by the Promotion of Access to Information Act 2 of 2000. In addition please state the reason you require this information and the capacity in which you request the data.
This entire process is subject to an approval process as per explanation below:
• 1st - the attached request form has to be completed and returned for capturing on the system.
o Once received it will be checked to verify that the requested information is available on our systems.
• 2nd - the relevant Program Manager for that specific product has to review and give approval for the request.
• 3rd - the Legal Officer has to review and give approval that from a legal perspective the requested information can be provided.
• 4th - the Information Officer has to review and give approval that the requested information can be provided.
• 5th - Finance has to invoice the requestor for the information.
• Lastly once Finance has verified payment the BI team can then generate and forward the requested information.
Please take note of the following:
• Delivery of information is subject to payment of an initial fee of R1350 excl VAT and R450 excl VAT per delivery thereafter.
• On this where applicable section E (d) states that: If you qualify for exemption of the payment of any fee, please state the reason for exemption.
• The requested information will only be forwarded onto you once payment has been received.
